first of all, dial-up modem could be really pain...
That modem how old is it??? Uninstall modem, remove him from motherboard and put it back in different slot...
Then start PC, and install driver for modem...
create a new connection and see what is going to happen and would you get BSOD again and also check can you open web pages that you could not open before...
Since you got new HDD, check cables... Maybe it is not hooked up properly...
But most likely you should buy new dial-up modem...
