If you can startup for 30 seconds or so, which is normal, hit Ctrl-Alt-Del and call up task manager. Under the Processes tab, find MSBLAST and terminate the task. You'll then be fine until you restart the machine, and you can download the fixes and removal tool.
I recommend you consider a firewall too, since even with the Microsoft patch, the worm can crash your machine if it has access.
