having reformatted and all that other stuff it is definitely hardware issue.

Does your motherboard have an on-board graphics card? Is it disabled? If not try it. If no on-board graphics then strip everything out (including souncard) except the graphics card, disable all peripherals in the bios and boot up. If the problem still arises then its the definitely the graphics card. If not, then one by one put your devices back in giving enough time to test them individually. It may sound tedious and drawn out but it is the only way to test a complex machine like a pc.

While your at it, you may want to consider upgrading your OS to 2000 or XP as your system spec is a very heavy load for an operating system which was developed five years ago- something never really thought of. This may be causing probs itself.

Get your hands on a cheap graphics card first(if no on-board card) in-case it is a Mobo issue.
