Issue changing NAT

A few days ago my NAT changed randomly from Open to Moderate for reasons unknown to me. After much research, I have changed all of my Netgear's settings to best suit my Xbox 360, from DMZ to Port Forwarding (88 and 3074). Under WAN setup, my NAT Filtering option is set to Open instead of Secured, but Xbox Live is still telling me that my NAT type is Moderate, as well as the game I own that can access that information. What needs to be fixed here?

Appears you are missing some ports in your forwarding for xbox

TCP 80
UDP 88
TCP/UDP 53
TCP/UDP 3074

have you been to shields up! and done port tests for these ports to see if they are open?
