Thanks for the info. My problem is that Web Easy does not have a code view option (maybe I am to dumb). I cannot complete the 2 steps below you listed.
7.Switch to Code View in FrontPage.
8.Paste your code between the <body></body> tags where you'd like your form to appear on your pag

I'm not sure if Web Easy has such an option, as my experience with this particular program is very limited. Keep looking around to see if you can find it, they've probably put it in an illogical place.

You can open the .html file in Notepad and paste the code block into the page there, you might have to experiment with the placement to get it exactly right. You can open the page in your browser locally to get an idea of where the form will be.
