Deleting/Moving/Pasting progress bar won't go away!

This very irritating problem just recently showed up on my computer. When I delete, move or copy-paste a file anywhere, the progress bar just gets stuck. The task itself gets completed but the progress window doesn't go away. When I hit "cancel" the window says "canceling task" but remains frozen. I'm forced to go into Task Manager and reset explorer.exe to get rid of it.

Any help will be appreciated. I've already tried rebooting and virus scanned the computer twice but to no avail....

Clean out temp files - Download CCleaner 3.04.1389 - FileHippo.com

Defrag HDD - Download Defraggler 2.03.282 - FileHippo.com

If problem continues, boot into SAFEMODE and try file copy. Tap F8 key repeatedly during boot-up.

Um, I ran both programs. It made my computer run faster but it didn't fix the problem at all. What exactly do you mean by file copy?
Oh yeah, and I forgot to mention, the search bar doesn't work.
Try the file copy in SAFEMODE. Are the results the same - hanging progress bar?

Run chkdsk /r

OK, the file copy works, the only problem I have is with moving files and deleting them. That's when the hanging progress bar happens. Also, trying to rename files causes explorer to crash. I already checked my disk but that didn't help. I'll run the HDD diag in a moment.
Check these 3 for Explorer.exe crash details -

1. WERCON -
START | type view | "View all Problem Reports" | 2x-click on line item for additional crash info

2. Reliability Monitor -
START | type perfmon /rel

3. Event Viewer -
START | type eventvwr.msc | Custom Views | Administrative Events

Alright, I searched through all three programs and there are no problem reports or crash details for Explorer.exe since the problem began. If Explorer.exe truly has a problem, it seems to be contained and not affecting Windows as a whole.
Nothing works, man... Went through all the programs and explorer.exe doesn't have any errors. Checked the disk and everything. What gives?
Did you run the HDD diags - starting with SeaTools for DOS?
