Assistance with making a Dialup Account CD

Hello,
I need assistance with creating a CD which I can give to people to configure Dialup networking for them.
Here is what I need the CD to do:
1) Auto configure dialup networking for the user with a telephone number I provide
2) Prompt for username and password
3) Work on Windows 95/98/2000/XP and possibly Mac if you know Mac.
4) Be easy for someone to use.

Possibly a "splash" page that is auto run when you insert the cd would be ok....and then they can click "install" etc.

If you can assist with this, I'd be happy to pay a small fee.
