Need some clarification on WSUS Offline Update

I understand how to uses WSUS to download updates for a given OS. My question is this-After an initial run of updates for a given OS do I have to download the same updates again on subsequent runs or does the program just add newer updates to the previously downloaded ones?
Where is the updateinstaller file? It is not in the list of extracted files from when I downloaded WSUS Offline Update. I'm guessing it must appear in the package with the downloaded updates. And finally, for what purpose would one create an iso of the updates? I can understand how one would want to slipstream the updates into a Windows install disk but just can't see how updates would get installed using an iso of nothing but the updates. Color me dense! Thanks y'all.

My System


updateinstaller.exe will appear in the client folder.

Use an ISO (from disk, flash drive or network storage) for unattended install of all updates. Very useful when updating several PCs at once.
