This is my problem/story. Around 6-12 months ago I found out my RAM card(s) were faulty or just wore out. I figured this out by doing loads of research and I decided to replace them. I bought 2 brand new RAM cards of the exact same kind. (2)Ultra 1024MB Pc5400 ddr2 667 mhz. And now 6 months later I'm having the same issue. I don't know if this is a voltage problem or just my RAM cards.

My question:
What is causing this and how do I fix it?

No it doesn't need brand name and model.

then one ram stick is knackered download memtest 86 burn to disc and with one ram stick installed boot from that disc and let memtest test for several passes then swap sticks/

your 12v voltage is very low and indicates your psu is on its way out. please replace ASAP as ultra power supplies are known for damaging equipment when they fail.

Infact it could be the reason your ram stick is knackered.
