please help, im not sure if its a trojan? or whatnot.

A few days ago, I started my computer and it had this problem. What happens is it loads all the way to the point that I can see my wallpaper, but does not load any of the icons or the start menu. It's completely blank except for the image I have for my wallpaper. i can use taskmanager to get through to any folders, enough to run programs and back up my things, but cannot open them.
i have read through many forums and have tried running explorer as a separate process, and also tried to find a file in regedit to delete, but that file couldnt be found.
ive previously used avg, but it seems not to be working so well anymore, so i downloaded malwarebytes and ran that through, and it supposedly got rid of the threats.
im not sure what to do anymore, please helppp!
