devices to use VoiP at internet cafes

I was at an internet cafe overseas and saw a guy with his hand over his mouth. I was thinking that maybe he was using VoiP. Are there USB devices and small earphone/microphones that let you use VoiP to call discretely like this gentleman seemed to be doing??? Any help is appreciated.

Is the Skype SMC WiFi Phone & FON Router for $159.00 a good deal? Do I need a wireless router? I'm overseas and do not want to pay for a connection. Maybe I should just use Skype with the laptop....
