Did you install the USB drivers? Does the modem appear error-free in Device Manager? How about doing this?
For 98 or ME, Start, Run, COMMAND to open a DOS window: --- or --- For 2K or XP, Start, Run, CMD to open a DOS window: Type the following command: IPCONFIG /ALL Right click in the command window and choose Select All, then hit Enter. Paste the results in a message here.

Try this Automated WINSOCK Fix for XP, reboot and test.
Any particular reason you have no service packs installed? You are wide open to problems without the security fixes.

While you've seen this modem work at another location, I think it's time to try another machine on the modem at this location.

Have you tried the new machine at another location? It's starting to sound like you have a hardware issue. I presume you ARE power cycling the modem whenever you change the attached device or interface, right?

Well, I suggest you solve the PCI bridge error, since that could be a key factor. What appears under Network Adapters in Device Manager?

And the Realtek Ethernet adapter has no errors?
Did you turn off the power on the modem, wait for 30 seconds, and turn it on again?
