BUGCODE_USB_DRIVER BSoD Problem
Hello. I am having serious issues regarding the Blue Screen of Death(BSoD). Ok, the problem originated when I aquired a PCMCIA adapter for my computer, so I could access the internet from my desktop. Now, whenever I use the Internet, I will occasionally get the BSoD, causing my computer to restart. This is especially a problem when I am downloading files from the internet or playing an online game. The title for the BSoD is "BUGCODE_USB_DRIVER", the error code 0x000000FE, and the parameters are:
parameter1 00000005, parameter2 8972f0e0, parameter3 10330035, parameter4 895882d8. According to the Miscrosoft developer network, the problem is "A hardware failure has occurred due to a bad physical address found in a hardware data structure. This is not due to a driver bug." So how do I know what hardware is the problem? How do I fix it? Can I fix it? |

![]() To figure out what file or piece of hardware is causing the BSoD I will need your minidump files to work with. Please follow the below steps for me to obtain your dump files. I would like you to go to:
Then I would like you to enter this into the address bar or go to this directory: C:\Windows\Minidump When successfully navigated to this folder I would like you to move all of these files onto your desktop and create a new Zip folder called "Minidump Files." Place your Minidumps into this Compressed Zip Folder and upload it to the TSF Server in your next post including more information on what has happened since my last post. The second method is that you can upload the same Zip Folder to another file sharing server like Microsoft Skydrive if you have a Windows Live Account so I can download it there to. |

I have looked at a few of the files, and they point towards USB Driver Issues. Please follow my below steps and see if any of them will fix your situation:
Please go to Microsoft Update and choose Custom as your update option. Then install ALL of the updates listed and please reply back here in your next post if you have an issue. Check all of the boxes and go through all of the tabs, making sure all of the updates are checked. Then go back to Microsoft Update once again to get all of your updates and hotfixes, until there are none left to obtain. ------------------------------------------------ Disconnect all USB Devices from the system and see if the Crashes stop. If they do, connect each device one by one, and see which one is causing the issue. ------------------------------------------------ It seems that you might have an issue with drivers on your computer so we will now do a check with the Windows Device Manager to make sure of this. Please follow the steps below: Windows XP Method
Then expand Universal Serial Bus controllers, and uninstall all of the USB Root Hubs by right clicking them and then clicking uninstall. Then restart for the system to reinstall the USB Hubs. ------------------------------------------------ After looking over extensively your computers situation I have found that one or more of your Windows Files have become corrupted or deleted. To combat this issue we are going to run the System File Checker on your system which is a built in Windows checking utility that checks and fixes corrupted Windows Files. Please follow these steps as best as possible and if you have an issue please post it back in your next reply: Windows XP Method

Thanks for the advise. I'll see what happens.
BTW, I previously tried disconnecting USB Devices in an attempt to find the bad one. None of them stopped the crashes, but I found something interesting. There are two specific USB Devices that must be enabled for the Internet to function, otherwise it can't connect for some reason. I never tried disconnecting all at once, but I couldn't even test the theory if I did, as I said it only crashes when using the internet. Last edited by Darth Cyrax; 04-06-2008 at 05:51 PM. |

I have Service Pack 2, and as for the USB Devices they are labeled as such in the Device Manager: Curitel PC Card Composite Device (UPD), NEC PCI to USB Open Host Controller, NEC PCI to USB Open Host Controller, Standard Enhanced PCI to USB Host Controller, Standard OpenHCD USB Host Controller, Standard OpenHCD USB Host Controller, USB Mass Storage Device, and USB Root Hub. (I have 5 USB Root Hubs)
FYI: The two devices that disabled the internet when disabled are the Curitel PC Card Composite Device and a USB Root Hub |

Sorry for being gone so long, but I'm back. I have done the first two steps you reccomedended and so far I have had no BSoD problems. I will report back I if have any problems, but if by Friday night I haven't had any crashes I'll consider the problem fixed.

Here are the recent minidump files. BTW, I have not benn able to do the third step you reccomended because I don't think I have a "Windows XP disk." I bought my computer from Cyberpower and it already had XP installed. I checked the disks that came with it, but none of them say "Windows XP."

One file that showed up was PTDWMdm.sys. From what I can find out it's a USB Modem Device Driver by DEVGURU Co,LTD. See if you can find out what it belongs to and then look for an update.

I tried disabling every device except those that my internet requires to run. The error still occurs, so does that mean one of the three undisabled drivers is the problem? Or is it something completely different? I don't really understand what is causing the problem.

peterdiva, I know what you're talking about but I don't know what to do. I did find a DEVGURU.com, but that has seemingly nothing to do with the USB Modem Device Driver.
techpro5238: I do everything you say but there is one "NEC PCI to USB Open Host Controller" and its corresponding Root Hub that must be enabled for the internet to work. Not sure what to do next. I you have any suggestions, please tell me. |

Do a search on your system for the drivers I listed. The following is the only other info I could find.

Ok. I think I found the site that my drivers came from, but it is in a different language. The site is www.devguru.co.kr/
