Win 2003 Standard / Fax Server Issues

We have just created a new fax server and application to handle about 900 faxes per day. Our server is running under Windows 2003 (HP Compaq 380DL) with four modems attached, two for send and rec and two for send only. These modems go through a four port usb hub. Microsoft fax service manager and console are installed. We have created a beskope application that sends word documents and recipient phone numbers to the "local" fax server through use of a windows service. (This servers polls for new fax jobs (setting in a MSsql DB) and sends them to the fax server.) We know our code works as we have conducted huge load tests, sending all jobs to one fax number and other tests using a number of fax numbers. During go live, the faxes end up at a number of destinations and the windows outbound routing ensures all for modems are busy sending faxes. The same recipient fax numbers tend to queue on the same modem (device) and this is desirable. We know that the correct phone numbers are sent to the fax server as they appear in the fax header of the tiff image. What we were surprised to find is that some faxes do go to the correct phone number, but others go to the wrong recipient. Its completely random, - its as if the system retains previous numbers from fax jobs and applies this to the next or the fax providers are holding onto numbers used previously.
