EEK! Word 2003 - Outline Numbering Disappears

I've searched the forums and found no posts about this issue, so here goes my first post:

I constructed a document with numbered paragraphs in the 1., 1.1, 1.1.1 format. I created the document first by making the outline, then applying styles I wanted to each level, then finally adding my text. Level 1 had 1., 2., 3., numbering, with small caps. Level 2, or 1.1, had gray shading and a line on the top and bottom for a border. Level 3 was 1.1.1, 1.1.2, with bold text. I was able to create a table of contents, update it as I made changes, and everything was working perfectly.

I e-mailed it to my co-worker, who said the numbering had disappeared. I opened the document, and not only had the numbering disappeared, the level 2 text had moved way over to the left side of the page, as if page setup had text starting at 0.25. Funny thing was, numbering was still in the table of contents, but when I updated the table of contents to see what would happen, the numbering disappeared from there, too.

This is a template from which all similar documents will be created, so, any help would be greatly appreciated. Thanks!
