Are you looking for a blank piece of graph paper?

In Excel:

Format your page settings so the margins are where you want them.

If so, set all your columns to 4 and all your rows to 15 (you may have to tweak these to get true squares).

Set all the cell borders to on, use whatever border settings you want, like 50% grey so the lines won't be over dark, or set every 5th or 10th line darker or thicker.

Change the view to page view and set the print area (page set up window, Sheet tab, first setting) to match one page of your spreadsheet. (for my sheet with .5 margins all around it's A1:T49)

Word:

Insert a table with the number of rows and columns you want. (Add an extra column and/or Row for labels if you want them.)

Highlight the entire table and open Table Properties.

On the Table Tab, open the Borders and Shading window and set the borders for your lines, colors, line widths, etc. (If you want heavier lines for index rows and columns You will have to select the cells next to the index lines and set them individually, just like in excel.)

On the Row tab, set the row height to Exactly 0.25" (for 1/4" graph paper) or what ever other setting you want. and un-check the "Allow Row to break across page" box.

On the Column tab, set the Preferred Width to the same setting you used for the rows. Be sure "inches" is selected in the "Measure in" setting.

If you want a label column and row: select the first column and change the column width to allow space for labels; select the bottom row and change it's height to allow room for labels; Select the 2nd through last cells of the bottom row and Format the Text Direction to Vertical. You can also select a group of cells and merge them (Table -> Merge Cells). This is important if you have small cells and need room for large labels, or if you want your labels centered on every other line rather than between the lines.

You can also format the cells so that the labels are positioned just over or next to the line they refer to and right up against the axis they define.

If you want to add index lines, select the columns to be indexed around (say the left 10 columns in a 0.1" grid) and set the Borders and Shading for the outside vertical lines to a thicker, darker line (like 50% grey at 1 pt) make sure you click both outer edge buttons to turn the sample cells borders off and on. Then set the inside border to a thinner, lighter line (like 25% grey at 1/2 pt) and click the inside vertical border line. Do this for each set of rows to be indexed. Next click the rows to be indexed and set the borders the same way, but this time only modify the horizontal lines. Feel free to vary colors, line thicknesses and any other characteristics you want.

This can be a time intensive process, so be sure to save frequently and write protect your finished document.
