Ms Word want header fixed so user not delete it.

I work in security job and i have created a investigation report using Microsoft Word 2000. Simple form with headers and text field (Attached)
I am interested in knowing how to make the green highlighted headers non-deletable fixed i.e headers form a template. How to make that as template i like to know. if its not clear pls ask for more clarity and i'll provide you it. cheers.

much appreciated in advance for any tips.

While you may have "fields" you want filled out, Word does not refer to them as "headers". A header (in Word) is text/logos that appear at the top of each page (without you having to paste them at the top of each page). What you need is a fill-in form.
