Is it my Graphics card?

Hi, I have a Phillips MT2400 that I have allways had problems with. It will always freeze and the monitor goes black when playing any game. It has done that from day one. But recently it has started to freeze all the time. I removed the graphics card give it a good clean and it seems to work fine now. But it will still crash almost immediately when a game starts. The graphics card is a Radeon x550, do i need to replace it?

What brand and wattage is the PSU. (Open case and look at label)

Post the temps and voltage from the BIOS.
