X1900 GT Crashing

Recently, every time I go to play a game (mainly Everquest) , they work for about 10-15 mins and then I get a crash. By crash I mean a black screen and the monitor goes into hibernation. I have to restart the computer.

Comp specs:
Pentium D CPU 2.80GHz
2.0 GB of RAM
Radeon x1900 GT

Voltages:
+12V: 11.31-11.42
Temp: 71C
CPU temp: 37C
Every time I run scan for artifacts on the ATITool it crashes and goes into hibernation.
I'm not sure what I need to do next.

Is the 12v reading from the Bios or a windows 11.31v is way too low for a 12v rail.

That reading was from Hardware Sensors Monitor. After a night of playing Sins of a Solar Empire with no problems the +12V ranged from 11.31 to 11.48.

Check it in the Bios or with a digital volt meter but below 11.7 can cause problems with video cards.
