Sound input/output mixed up

i have an odd problem. what happens is whenever i use the microphone at all (record in the recorder, play online games, etc...) i can here what's coming out of the output.
For example, if i play music in the background and then i press the talk button and talk on a game, then my music is played for the time that my talk button is held down. it is also like this for when i record stuff. i can hear the stuff playing.
I know it's not because my microphone is by my speakers because it does the same thign when i am using head phones.

check you have the leads plugged into the correct connections and have not swapped any over

I checked and doulbe checked over and over. they are plugged in right. do u have any other suggestions on what it could be?
