Help with ImageQuest L50c

Hello everybody. I've been at this dodo bird search for any kind of information on my monitor. 1st. thing is that there is only two connection on the rear of the monitor. One is the d-sub 15-pin for VGA and the other appears to be a 6-pin mini din similar to a ps/2 keyboard plug. Thats all. Now i assume this is its power source. my question is is this correct? Does anybody have a picture of how the connections are? Can i just make my own? Power Required is DC 12v/5v 2.0a/2.0a. Can someone clarify what the slashes represents? Any help would be extremely appreciated. Thank you.

This breaks down to:
12v @ 2amps
5V @ 2amps

thanx. I was doing a little browsing and i stumbled apon this info http://www.lcdpart.com/doc/adselect.html Can someone please help me if possible a simple instructions on how to make this 4 pin to a 6 pin.? Has anyone ever came across a plug like this before? since the monitor being an analog type, would cable type matter, which ones of these would be best. VGA, SVGA, MultiSync cables, Analog cables? ;Is it possible that it can damage the lcd monitor? Sorry if i am being a brat, i just wanna see a picture of all the acc. that came with the monitor packed from the manufacturer. :P Somehow i manage to confuse myself.
