At this point, there is no knowing why it is doing this. If swapping the cord didn't help, then next thing is to try a different modem on the same Mac, if it fails, then try a fresh install of the OS. If it still fails, then the motherboard is bad. So, if you don't have access to another modem, then you will most likely need to buy one. If you happen to be near any Apple store, take it in, and they can try a modem right in the store.
