fake alerts amongst other things

hey everyone would appreciate some help please, have removed several bits of spyware and still havin probs so think i've missed a few things. got the fake alerts and lots of pop-ups, takes about 20+mins to log onto OS. never used HJT before but here is the log, cheers (edit: when i try booting into safemode i log in and it stics on the black screen saying safemode around edge left it up2 half hour no change. command prompt one works ok tho)
