Tested on another system by removing icons on taskbar via "unpin" option, rebooted and it's gone.
Sounds like the other system is potentially infected.

nope, it actually came back after finding one malware and rebooted but still has issue. Anyone seen this before?

 

make sure the taskbar is unlocked before deleting/unpinning.

 

Hi, could you right click the icon select properties and open file location , let us know what you see.

 

been informed that the renaming the iconcache as I'd suggested has resolved the issue.
They never rebooted as I had indicated.
