What is the spec of your system including drive models and PSU info ? (you will need to read that from it's sticker on the side of it inside the case).
Why did you need to format it ?
how did you format it ?
Have you touched\changed ANY hardware in your system just before this started ?
And were there any signs of this or similar starting to show before this happened ?
Sorry for so many questions but without such info we are working blind.
