At 13.1V, your +12V rail is way over spec, which means you have a faulty PSU. It's always best to stick with high-quality brands such as Silverstone, Seasonic, OCZ, Antec, FSP - the lower quality brands are more likely to under- or over-power your system, which can cause problems like you're having, as well as damaging components.

So, first step: replace the PSU (preferably with a better brand, if you can).
