Definitely sounds like the head is making contact with the platter (which it shouldn't) and/or the motor is borked. You can download one of a plethora of free SMART reading programs out there and see any recorded errors made in the hard drive's SMART log.

In any regard if you have any access to the hard drive, you should backup your data as soon as possible and consider having the hard drive replaced.
