I could not complete all of your instructions that you posted last. There was no 020 - winlogon notify: dmsext - C:\WINDOWS\SYSTEM32\dmsext.dll. There was however
"020 - winlogon notify: dmsext.dll (file missing)" and there were two listings of "020 - winlogon notify: navlogon - C:\windows\system32\navlogon.dll
I thought I should tell you this before proceeding in any direction. I am posting this by another computer, so I have my infected computer halted right before the "fix checked" step.
