I just want to clairfy one thing before I proceed with the fixes you suggest.

You say:

"Run a scan in HijackThis. Check each of the following if they still exist and hit 'Fix Checked' after you check the last one:"

- Does this mean I should make sure each in that list have been REMOVED? - not sure what I'm supposed to do with that list.

Everything else seems clear to me - thanks for the help!
