You'll have to have the cable box on a static IP address, or use the DHCP reservations capability to insure it retains the same IP address. Then you would port forward the required ports to access the Cable box.

Of course, the exact make/model of all of these units would be real helpful here, as well as exactly how this cable box connects to the router.
