What were the voltage readings of the +12, +3.3 and +5v when running the test?

.02 is not much of a fluctuation and if the readings were within spec that would not be the problem.

This is very hard to diagnose without swaping parts the best thing you could do is barrow a psu to try in it at least that might eliminate that as a possibility.
