So I ran a chkdsk d: /f in the cmd.exe. As soon as it ran it showed:

The type of file system is FAT.
An unspecified error occured.

And I have tried to perform system checks/repairs with partition magic and it would come across errors also. So it seems the windows and 3rd party softwares can't access the drive correctly..
