Heparin's used frequently in hospitals on a daily basis; probably millions of times per year around the world. Two isolated instances of mistakenly giving the wrong drug dosage sounds more like a major screw up by the doctor or nurse, rather than a "problem" with the drug company and/or it's label methods.
