As you can see, that scan came up clean.
We missed an old McAfee entry. Open HijackThis and do a scan only. 'Check' and click 'Fix Checked' for the following item:
O4 - HKCU\..\Run: [MSKAGENTEXE] C:\PROGRA~1\McAfee\SPAMKI~1\MSKAgent.exe
Close HijackThis.
